Question about NJ traffic ticket?

So I’m 17 with a provisional license. I was already ticketed about 5 months ago for going 95 in a 55 on the turnpike, so this is my second offense.

It was slightly past curfew (11:05 maybe) and I was heading home (about 2 miles away) and I got to a traffic light. It was turning yellow, and I was already late home and past the curfew, so I tried to go through it. Turned out a cop was at the intersection and saw the whole thing.

He pulled me over, and only gave me one ticket for failure to observe signal(in reality I was breaking about 4 laws, so he was really nice).

Problem is that I’m not sure this ticket is valid. There is nothing written in the "prefix" box at the top of the ticket, and the box "court date required" is not checked, although he did set a court date for me.

What do I do? I can’t view the ticket info online because of the lack of a prefix.
